Covid Statement and Policy

This is an overview of our COVID policies for patients who visit us in-office for appointments. Please read this carefully in preparation for your next visit.

Covid Statement

In regards to the current and continued viral outbreaks, our first priority at Beverly Medical Center remains the health and safety of our patients and their families, as well as our staff. Our goal is to continue to care for patients in a safe and effective manner. We are offering in-person care, in addition to phone and video appointments for those who would prefer a telemedicine appointment at this time. When you enter the building your temperature will be taken. Before you come for your appointment or when you enter the building we will ask you to review, complete and sign a form containing the following questions:

  1. Have you been tested for COVID within the last 14-30 days?
  2. Have you had contact with anyone that has had a confirmed COVID case within the last 14-30 days?
  3. Have you or anyone in your household been quarantined of tested for COVID in the last 14-30 days?
  4. Have you traveled outside of the US in the last 4-8 weeks?
  5. Have you recently traveled in the US?
  6. Have you been in contact with someone who has traveled inside the United States, outside of the United States and is now sick or was sick?
  7. Have you been told by a public health official that you may have been exposed to coronavirus (Covid-19)?
  8. Are you experiencing any of the following symptoms:
  • Fever
  • Sore Throat
  • Vomiting
  • Muscle Pain
  • Cough
  • Severe Fatigue
  • Diarrhea
  • Hives and Itching
  • Runny Nose
  • Nausea
  • New onset loss of taste/smell
  • Chills
  • Abnormal Headache

We wish to remind everyone that it is the policy of Beverly Medical Center that everyone entering the building must:

  • Wear a mask that covers your nose and mouth
  • Fill out the Covid 19 Statement Form honestly and completely
  • Have your temperature taken
  • Practice social distancing and good hand washing

If you don’t have a medical mask we will give you one when you arrive. 

If there is any medical reason (such as severe COPD) that you or anyone who will be accompanying you will not be able to follow the mask policy, please let us know ahead of time so we can make alternative arrangements for you. 

To encourage limited capacity of individuals in the building, we ask that you do not bring others with you unless it is absolutely necessary for safety and mobility reasons (a.k.a caretakers, parents or guardians of young children, etc.).
